The primary purpose of epidural analgesia is to relieve maternal pain and maximise the chance of a good outcome for both mother and baby. The use of epidural analgesia to promote the birth of a healthy baby will increase in future because there is clear evidence that epidural analgesia reduces the risk of caesarean section, which is a major reason for operative birth. Epidural analgesia does not increase the risk of the foetal adverse outcome. In short-term studies, epidural analgesia is associated with a lower risk of shoulder dystocia and operative birth, and a shorter length of labour. It has an influence on the course of labour as the risk of instrumental vaginal delivery is reduced. The optimum time for giving epidural analgesia in labour appears to be before 30 weeks' gestation and after 36 weeks' gestation. For the second stage of labour epidural analgesia has no role to play. There is no difference in the length of the second stage of labour, but more epidural analgesia is required. Concerns about epidural haematoma and recurrent dural puncture are unfounded as the literature does not show a significant increase in the risk of post-dural puncture headache. Epidural analgesia is associated with a modestly increased risk of caesarean section. Women may be more likely to want epidural analgesia if they have a longer labour. In this case, women who have a shorter than usual labour are at increased risk of operative birth and should be offered a trial of epidural analgesia before a decision is made. In future, we may be able to tailor epidural analgesia to optimise the trade-off between reduction in the number of caesarean sections and increased operative birth.
